COVID vaccine exemption certificates

Provision has been made for patients who, for medical reasons, are unable to be vaccinated against COVID-19.

Patients who are eligible can apply for an exemption certificate which, if granted, will provide them with an NHS COVID Pass.

How to apply:

Patients should call 119 and request an NHS COVID Pass medical exemption form. Registered carers can apply on behalf of the person they care for.

Patients will need to provide:

  • Full name
  • Date of birth
  • NHS number
  • Name of GP or specialist clinician / hospital consultant.

The completed form should be returned to the patient’s registered GP or hospital consultant, who will clinically review the application. Your GP or consultant will come back to you if they need more information or any further clarification.

Once your application has been reviewed and processed, you will be automatically notified of the outcome by post. The clinical decision on the medical exemption is FINAL and cannot be appealed.

Please note: GP practices are unable to provide the application form. Patients must register with 119 first.
